On your first visit, an initial evaluation will be performed to help diagnose and develop a personalized Integrated Treatment Plan to lead you on your path to recovery. During the assessment, a Chiropractic Physician will take a detailed report of your musculoskeletal complaints, as well as discuss how these are affecting your daily activities. The doctor will also assess your joint integrity, range of motion, strength, and flexibility, and will focus on any movement impairments. If your personal physician has already given you a prescription for physical therapy, a physical therapist will complete your exam and begin your treatment.
Every patient is unique. Your candid input will guide preparation of a comprehensive Integrated Treatment Plan, a roadmap to achieve your goals, and is vital to the success of our treatments. Based on your overall health, treatment needs and goals, history, and examination, our team will create a comprehensive, Integrated Treatment Plan to address your needs.
Your Affinity Integrated Treatment Plan will include:
- A summary of your chief complaint.
- Precautions to take during and outside of treatment sessions.
- Diagnosis/Area of Concern
- Medical history, including any complicating factors.
- Frequency needed for care
- Functional Goals: Medical Treatment, Physical Therapy, Home and Self Care. Functional goal example: “By the end of treatment, the patient will be able to walk for up to 40 minutes at a fast pace without pain.”
Treatment plan, including chiropractic adjustments, soft-tissue mobilization, therapeutic exercises, or other treatment modalities to meet your recovery needs and goals. The frequency and duration of each treatment type will be specified in the plan. Functional goals for your activities of daily living.
